The Tourist Card is a single-entry visa.
Yes you need a Tourist Card. If you are a US national OR if your cruise departs from the USA, you will need a pink card. Otherwise you will need a Green Card.
If you have a direct flight from the USA to Cuba OR if you are a US national, you will need a Pink Card. If your flight has a layover outside the USA (and you are not a US citizen), you will need the Green Card.
Children require a Tourist Card, even if they are registered on their parents’ passports.
A Cuban Tourist Card and a Cuban Visa are different documents. Only a few African and Middle Eastern countries need a Cuban Visa instead of just a Tourist Card. If you’re coming from America, Australia, New Zealand or Europe but you will need a Cuban Tourist Card. However, Cuba Tourist Card is often referred to as a visa.

The Card does not expire before traveling, this means that you can travel at any time after your purchase. Once you enter Cuba the card has a 90-day validity.
You can apply for as many as you want. Bear in mind that each card will allow you to stay for up to 90 days.
The Tourist Card can be extended for an extra 90 days while you are in Cuba. The maximum stay is therefore 180 days for all nationalities.
Let’s say your itinerary is USA – Panama – Cuba. In this case you will need a green card to enter Cuba since the last leg of your flight is via a non-US airport

If you are travelling to Cuba from the USA, your trip will need to fall under one of the approved general licenses for 12 categories of travel. The 12 categories of authorized travel to Cuba are: family visits; official business of the U.S. government, foreign governments, and certain intergovernmental organizations; journalistic activity; professional research and professional meetings; educational activities; religious activities; public performances, clinics, workshops, athletic and other competitions, and exhibitions; support for the Cuban people; humanitarian projects; activities of private foundations or research or educational institutes; exportation, importation, or transmission of information or informational materials; and certain authorized export transactions.
